During the period 5-7 May 2009, the Petroleum Safety Authority Norway (PSA) conducted a major accident audit of Gassco AS at the Kårstø facility. We identified no non-conformities in relation to the regulatory requirements, but some potential improvements were noted.
Background for the audit
The audit was conducted on the basis of the Regulations relating to measures to prevent and limit the consequences of major accidents in activities involving hazardous chemicals (the Major Accident Regulations), Temporary Regulations of 19 December 2003 relating to safety and working environment for various petroleum facilities on land and associated pipeline systems (temporary regulations) and Regulations relating to systematic health, environment and safety work in activities (the Internal Control Regulations).
Purpose of the audit
The purpose of the audit was to verify compliance with applicable regulations in areas which influence the risk of major accidents. This includes:
The activities' identified risk scenarios, incl. risk assessment and analysis
Preventative and consequence-reducing measures in connection with major accidents, incl. emergency preparedness and drills
Reporting and follow-up of incidents and accidents
Technical and operational integrity, including safety-related routines and maintenance.
Result of the audit
The Kårstø facility has had a positive development over the last few years as regards awareness and systematic work to reduce major accident risk.
Several important measures have been implemented. Awareness and understanding of contribution to risk and control of these were evident among all the interviewees. The interviews gave an impression of systematic work throughout the organisation.
Potential improvements:
There is no systematic approach to the verification of measures implemented following incidents and investigations.
There is no systematic review of implemented measures to see whether the desired effect has been achieved.
There is also no systematic analysis of the incidents recorded at the facility.
The responsibilities in the organisation are ambiguous as regards technical and professional responsibility.
